Position Summary

We have an exciting opportunity with the Home Health program in Trail! If you are a self-directed team player who enjoys working with an interdisciplinary team approach and are a keen learner with strong knowledge and experience in geriatrics, case management and chronic disease management, this could be the position for you!

This clinician role is a dual role home health time limited and continuous care position. If you have an interest in helping clients become as self-sufficient as possible this position may be a fit for you. The continuous care component includes case management with client and family dynamics as well as social dynamics with the client in the community.

As the successful candidate, you will support the client’s achievement of safe realistic and reasonable goals by assessing, planning, coordinating, organizing, implementing, evaluating and transitioning nursing and other care services for the client. In collaboration with the client, family and other care providers you will develop, implement and adapt a care plan, deliver direct care, assess service eligibility and provide care management to help clients manage their own care and navigate through the various services available within the community setting. Working in consultation with other system partners, you will base your practice on nursing science and related content from other sciences and humanities.

Casual employment includes: short-notice, on-call availability; workload coverage during a temporary absence; and/or providing relief coverage in a specific position. Graduation from an approved school of nursing with current practicing registration with the British Columbia College of Nurses and Midwives (BCCNM).

Candidates must also hold a valid B.C. driver’s license. Demonstrated knowledge and skills within a complex, geriatric patient population and chronic disease management.

Experience with wound care and palliative care an asset. Sound computer skills including experience with Meditech and Excel an asset.

A gerontology certificate would be an asset.
